Credentials Dr. Washak is a graduate of Texas College of Osteopathic Medecine. After an internship and residency in General Surgery and Otorhinolaryngology, Dr. Washak completed a cosmetic surgery fellowship. Active participation in Desert Storm at Walter Reed Army Hospital followed by a residency in plastic and reconstructive surgery in Des Moines, Iowa. Dr. Washak is trained in the full spectrum of cosmetic and reconstructive surgery procedures. He is board certified by the American Osteopathic Association in Ocolaryngology and Facial Plastic Surgery, Member of American Society of Plastic and Reconstructive Surgeons, Diplomate of Osteopathic Board of Medical Examiners, member of American College of Osteopathic Surgeons, American Academy of Cosmetic Surgeons, and the International College of Surgeons.

- By eliminating billing expenses, we can help lower your health costs. Therefore, payment is requested at the time of your visit.
- The agreement of your insurance company to pay your medical expenses is a contract between you and your insurance company. This means that you are responsible for co-payments and any uncovered or unmet payments on your account. Our office will provide you with sufficient information to help you file your own insurance when required.
- You must bring with you authorization forms signed from your family doctor if required under your insurance plan
- Any Cosmetic Surgery procedures must be prepaid before a date can be scheduled.
